ABOUT THE ROLE 

The Care Coordination Manager plays a pivotal role in overseeing and improving the care coordination program and ensuring that patients receive holistic support and seamless care. This position involves overseeing a team of Care Coordinators who work closely with patients, clinical teams, and other staff to identify needs and facilitate connections to resources, support, and services available in the community. The Care Coordination Manager ensures that communication between all parties involved in patient care is maintained at the highest level, fostering collaboration and minimizing interruptions in care. The manager will guide the team in developing patient-centered plans and approaches, utilizing community resources and building individualized treatment plans, using data to drive the team toward company objectives. 

 

 

WHAT YOU’LL DO 

  • Oversee and manage the people-related aspects of the department, including fostering employee and team development, driving performance and development initiatives, building and sharing best practices, providing coaching and feedback to support growth, ensuring team retention, and cultivating a positive and collaborative team culture. Manage and oversee a team of Care Coordinators who collaborate with patients, providers, ancillary departments within the company, and community agencies to maintain accurate and timely information exchanges between all entities involved in the patients’ care.   
  • Ensure the provision of Care Coordination services to patients, including making referrals to appropriate community resources to address barriers such as housing stability, legal challenges, access to transportation, physical health needs, economic stability, community integration, and access to social support.      
  • Coordinate the daily operations of the care team members to ensure quality and timely delivery of care.  
  • Delegate patient assignments to Care Coordinators and ensure the team welcomes and orients patients the program as well as identify needs, strengths, goals and barriers.   
  • Collaborate with operational and clinical leaders to implement group programs and lead team in facilitating groups on topics relevant to patients served.  
  • Ensure the team obtains appropriate Releases of Information (ROI’s) for each community referral that is completed and conduct follow-up for referrals made to each resource.   
  • Assist and support the team in coordinating community referrals, scheduling and attending appointments, and securing transportation. Guide the team in addressing barriers for attending Array appointments.    
  • Facilitate placement for alternative or higher level of care as agreed upon with the Interdisciplinary Team. Ensure contact is maintained with identified placement sites to provide continuity of care upon discharge.     
  • Maintain effective communication with patients, providers, team staff, and ancillary departments within the company and community.    
  • Build, maintain, and audit processes and workflows to improve operational efficiencies and ensure appropriate delivery of care to all patients.  
  • Lead and direct the people-related aspects of the Care Coordination team, including fostering employee and team development, driving performance and development initiatives, building and sharing best practices, providing coaching and feedback to support growth, ensuring team retention, and cultivating a positive and collaborative team environment.  
  • Outline and manage productivity targets for each care coordinator based on appropriate patient volumes.  
  • Interview, hire, and train additional team members as necessary to ensure the team is adequately staffed and capable of meeting patient needs.  
  • Attend regular meetings, including but not limited to Interdisciplinary Teams and external constituent meetings, completing all required documentation in Array systems.    
  • Facilitate staff meetings, providing updates and feedback and promoting team collaboration.  
  • Ensure timely and accurate data entry into all applicable Array systems by the team.  
  • Maintain excellent level of documentation and note keeping standards.    
  • Adhere to compliance provision regarding patient confidentiality.  
  • Provide crisis intervention as necessary.   
  • Lead team in developing community relations strategies to include regularly engaging outside resources.   
  • Develop rapport with patients through supportive, treatment-plan-based conversations.   
  • Maintain individual schedule of assigned patients.   
  • Performs other duties as needed 

 

WHAT WE’RE LOOKING FOR 

  • Bachelor’s Degree required, masters strongly preferred   
  • 5+ years experience in a Case Management/Care Coordination leadership role in behavioral health setting, preferably in a multidisciplinary telehealth setting 
  • 4+ years experience in crisis management, preferably in a telehealth setting 
  • Proven experience building and implementing care coordination strategic plan for behavioral health 
  • Strong understanding of the challenges within the behavioral health space and the ability to build and implement creative solutions  
  • Proven experience navigating and documenting in an EMR system   
  • Knowledge of assigned market community resources, networking, and referral processes    
  • Experience in group facilitation   
  • Excellent written and verbal communication, organizational, and customer service skills   
  • Ability to work independently with minimal supervision   
  • Mental stamina for problem solving, including managing stresses of a mental health service program   
  • Ability to prioritize multiple tasks, handle multiple tasks simultaneously and prioritize based on severity of need   
  • Ability to document, recall, and explain detailed training, conversation, or events, and apply oral/written instructions, as needed   
  • Ability to speak and hear sufficiently to communicate in various forms of spoken and written communication including virtual/telemedicine service delivery   

 

POSITION LOGISTICS & PHYSICAL REQUIREMENTS 

  • 100% remote position 
  • Required to have a private office space in home or elsewhere for confidentiality 
  • Constantly operate at a computer and other related peripherals such as a keyboard, mouse, headset, etc. 
  • Must remain in a stationary position most of the time working.  
  • Frequent communication via remote meeting software, softphones, or cell phones.  
  • Predominantly working in the employees’ home, indoor, office setup. 

 
 

COMPENSATION AND BENEFITS 

  • The pay range for this role is $70,000 - $78,000 annually. This information reflects the anticipated salary range for this position using current market data. The final offer will be determined based on an individual's skills, years of experience, education, capacity, licenses, and other job-related factors permitted by law. 
  • Medical, dental, and vision insurance, employer paid life & long-term disability insurance, additional voluntary benefits include short-term disability, voluntary employee, spousal and child life insurance, accident, critical illness, hospital, or confinement insurance and flexible spending accounts (FSA) and health savings account (HSA) contributions 
  • 15 days of accrued PTO annually, plus 8 paid holidays and 3 floating holidays
